MODEL RELEASE

I understand that Canine Link, Inc. (“Link”) is a not-for-profit corporation devoted to providing therapy dog services at hospitals, schools and other institutions. I understand that Link believes that my photograph, video or similar material would help Link advertise to promote and further its mission, and I also understand that I do not have to agree to this Model Release in order to benefit from Link’s therapy dog services.

I give Link permission to photograph, video or otherwise record my image, which may also include the images and/or voices of Link therapy dog volunteers and their dogs and/or voice, and I grant Link all rights to use those photographs, videos, other images and recordings in any medium for educational, promotional, advertising, or other purposes that support Link’s mission.
